Shirley was born May 24, 1927 in Chicago, Illinois, and passed away October 15, 2015, In Dallas, Texas. She is preceded in death by her parents, Stanley and Clara Samuelson, her Brothers, Dr.Gene Samuelson and Lawrence Samuelson, as well as her Sister-in-law, Joan Samuelson. Shirley is also preceded in Death by her Husband Klemens R. Formeller and her First Husband John G.Reba. She is survived by her Son, John Jr, Sister-in-law Janette Samuelson and many of her loved Nieces, Nephews and their families. Shirley received her undergraduate degree from Northern Illinois University, where she was a Sigma Kappa, and graduated Cum Laude with a Masters degree from the University of Arizona. A long time resident of Northbrook, Illinois, She retired as the head of the Mathematics Department of Glenbrook North High School. In retirement she enjoyed her membership in the Friendship League where she was active in Bridge, Bowling and Writing. Shirley was a Charter member of Gloria Dei Lutheran Church of Northbrook. A memorial service will be held at 11:00 am November 7th 2015 at Gloria Dai Lutheran Church 1133 Pfingsten Road, Northbrook, IL 60062. There will be a lunch following the service.
